Why was BridgeTrack created?

BridgeTrack was developed after recognizing a common gap in the mental health continuum of care.

 

Providers often encounter individuals who are clinically appropriate for outpatient therapy but would benefit from more support than traditional weekly sessions can provide.

 

BridgeTrack offers a structured therapy program designed to help those clients build momentum, strengthen practical skills, and transition forward with greater confidence.

What is BridgeTrack™?

BridgeTrack™ is Accedence Counseling's structured short-term therapy program designed to provide additional support during times when weekly therapy may not feel like enough. Whether you're transitioning from inpatient, Partial Hospitalization (PHP), or Intensive Outpatient (IOP) services, or you're looking for a more focused period of growth, BridgeTrack provides a structured space to build skills, practice new responses, and become more fluent in understanding yourself.

Who is BridgeTrack for?

BridgeTrack may be a good fit if you:

  • Are transitioning from inpatient, PHP, or IOP services.

  • Need more support than weekly therapy currently provides.

  • Want a structured, goal-focused therapy experience.

  • Are motivated to actively participate in your growth.

  • Want practical tools you can continue using after the program.

Your clinician will help determine whether BridgeTrack is the best fit for your current needs.

How long is the program?

Most clients complete BridgeTrack in approximately four weeks, although individual timelines may vary.

Because life happens, some clients may complete the program in up to five weeks if scheduling adjustments become necessary.

How often will I attend sessions?

Most clients participate in approximately 10 therapy sessions during BridgeTrack.

Sessions are scheduled in advance and gradually decrease in frequency throughout the program as you gain confidence using your skills independently.

Why are all of my appointments scheduled in advance?

BridgeTrack follows a structured schedule to help create consistency and support your progress.

Scheduling appointments ahead of time allows you and your therapist to focus on your goals instead of scheduling logistics while also helping maintain the rhythm of the program.

Can I reschedule appointments?

Yes.

If you need to reschedule, please contact your therapist or the practice as soon as possible.

Appointments will be moved to the next available time that supports your treatment plan and your therapist's availability.

Can I schedule my own appointments through the client portal?

No.

BridgeTrack appointments are coordinated through the practice to maintain the structure and pacing of the program. This helps support your progress and allows us to monitor your treatment plan throughout the program.

What happens if I miss appointments?

BridgeTrack is an active participation program.

Practice policies regarding late cancellations, no-shows, and applicable fees remain in effect during BridgeTrack.

Frequent missed appointments may affect your ability to successfully complete the program.

Will I have homework?

BridgeTrack uses Experiments rather than traditional homework.

 

Experiments are opportunities to practice new responses, gather information about yourself, and apply what you're learning between sessions. They are used in BridgeTrack to help you discover what works for you rather than trying to perform perfectly.

Experiments are not intended to be completed perfectly—they are opportunities to learn.

What are Fluency Tools?

Fluency Tools are the worksheets, exercises, and resources you'll use throughout BridgeTrack.

These tools are designed to help you better understand yourself, organize your thoughts, practice new responses, and continue using what you've learned long after the program ends.

Examples include:

  • SMAART Goal Tracker

  • Neutral Thinking

  • Response Cycle

  • Experiment Log

  • Bridge Maintenance Plan

Future Therapy Exploration Areas

What happens after BridgeTrack ends?

At graduation, you'll receive your personalized BridgeTrack Graduation Packet, including:

  • Your completed Fluency Tools

  • Bridge Maintenance Plan

  • Graduation Certificate

  • Graduation Letter

  • Recommendations for your next steps

You and your clinician will also discuss options for continuing your care, including:

  • Ongoing therapy

  • Transitioning to another provider

  • Psychiatry referrals

  • Community resources

  • Taking a break from therapy

The next step is yours to choose.

Can I continue seeing my BridgeTrack therapist?

In many cases, yes.

If you'd like to continue working with your BridgeTrack therapist, we'll make every effort to accommodate that preference based on therapist availability.

If another clinician would better support your ongoing goals, we'll help coordinate that transition.

Does BridgeTrack accept insurance?

BridgeTrack uses standard outpatient therapy sessions, and many insurance plans provide coverage.

Coverage, deductibles, copays, and coinsurance vary by plan.

Our team will verify your benefits before you begin the program whenever possible.

What if I need more support than BridgeTrack provides?

Your safety and well-being are always our highest priority.

If you or your clinician believes a higher level of care would better support your needs, we'll discuss those recommendations with you and help coordinate referrals whenever possible.

BridgeTrack is designed for clients who are appropriate for outpatient treatment.

What if I complete the program and want to come back later?

You're always welcome.

Some clients choose to continue weekly therapy after BridgeTrack, while others return in the future if another structured period of support would be beneficial and clinically appropriate.

Our goal is to remain your Resource for Life by providing support when you need it most.

What if I'm nervous about starting BridgeTrack?

It's completely normal to feel uncertain about beginning something new, especially if you've had difficult experiences in the past or you're unsure what to expect.

BridgeTrack is designed to meet you where you are.

You don't have to have everything figured out before you begin.

Your clinician will guide you through the program one step at a time, and together you'll determine what feels realistic, meaningful, and supportive for your goals.

For Providers

What types of clients are appropriate for BridgeTrack?

BridgeTrack is designed for adults and adolescents who would benefit from more therapeutic support than traditional weekly outpatient therapy but do not require a higher level of care. Each referral is individually assessed to determine whether the program is an appropriate fit.

How do I know if BridgeTrack is the right referral?

BridgeTrack may be appropriate when a client is clinically stable for outpatient care but would benefit from additional therapeutic structure, accountability, and support while working toward their treatment goals.

If you're unsure whether BridgeTrack is the right fit, we encourage you to schedule a provider consultation to discuss your client's needs.

Does BridgeTrack replace a higher level of care?

No.

BridgeTrack is not intended to replace inpatient, partial hospitalization (PHP), or intensive outpatient (IOP) treatment when those levels of care are clinically indicated.

Instead, BridgeTrack provides a structured outpatient option for clients who need more support than traditional weekly therapy while remaining appropriate for outpatient care.

Can I refer a client who is currently seeing another therapist?

Yes.

Depending on the client's circumstances and treatment goals, we may be able to coordinate care with the client's existing therapist or discuss the most appropriate treatment plan during the intake process.

What happens after I submit a referral?

After receiving your referral or consultation request, our team will review the information provided and contact the client (or you, if appropriate) to discuss next steps.

If the client appears to be an appropriate fit, an intake assessment will be scheduled to determine whether BridgeTrack is the best treatment option.

Will I receive updates about my referred client?

With the client's written authorization, we are happy to collaborate with referring providers and share relevant treatment updates when appropriate.

Our goal is to support coordinated care throughout the client's participation in BridgeTrack.

How long do clients participate in BridgeTrack?

BridgeTrack is designed as a structured therapy program rather than ongoing treatment.

The length of participation varies based on each client's needs, progress, and treatment goals. Recommendations are individualized during the assessment and throughout the course of treatment.

What makes BridgeTrack different from increasing therapy sessions?

BridgeTrack is intentionally structured to help clients move beyond insight by identifying what works for them, practicing new strategies, and building confidence through implementation.

Rather than simply increasing appointment frequency, the program provides a structured therapeutic experience focused on meaningful, sustainable progress.

What if I'm unsure whether a client is appropriate?
